Male - nf.
Female-prof or nf. Dont degrade urself by going with factions for either gender.

Make a fem and go with proph. Make a male and go with proph. Prof char design for both gender's is best out of any of the campaigns. But if you wanted to just cut to the chase, take that char to factions or NF, cuz that's where you'll get some good elites [Visions of regret] for example, and they're both quicker campaigns than proph. Although if you don't mind the slower pace and more content, then finish proph.

For PvE go NF. Get Heros and some good elites for PvE.

I don't know about the gender, but for campaign, I would say go Nightfall to get [[Cry of Pain] and access to heros.

Male or Female: How much does being asked to be some 12 year old's online girlfriend bother you?
Campaign: What do you want to look like, unless your making a perma-pre(in witch case this wouldn't be an issue) you get access to all of the same content. I personally like nightfall mesmers but its noob island sucks(will never make a nightfall pve char again.)

Character selection, gender, campaign selection etc... is all personal choice. It is not something someone else can tell you and you will automatically think it's the best choice for you. One person may enjoy starting in Prophecies, but you may find this too slow...so what have you gained there?
The only thing you need to know is that Prophecies has slow levelling and it takes a long time to reach the attribute quests. Factions is very fast in terms of levelling up. Nightfall is somewhere in the middle I would suggest.
Basic questions can be answered by consulting the stickied guide. If you have further, more specific, questions please feel free to post back here.
In short, we cannot build your character for you.
